Acknowledge the Financial Responsibility Statement

All new students must acknowledge their financial responsibility for the education they will receive at Stony Brook. To do so, please log into SOLAR and complete the following instructions:

1. Log-in to SOLAR
2. Click on "details" under the "Holds" section in the upper left corner of your home screen.
3. Click on the Hold Item labeled "Financial Responsibility Statement"
4. Click that you acknowledge the Financial Responsibility Statement and this hold will be removed

You must complete the above steps before you arrive at orientation.

Complete the Immunization Form

Complete the Immunization Form and return it two weeks before your orientation date. This form must be signed by your physician. If you are not 18 years old yet, then you must also have your parent or guardian fill out the Under 18 Consent for Emergency/Medical Treatment section.

Complete the Meningitis Response Form

All students must read the medical information about meningococcal meningitis, and complete and return the Meningitis Response Form two weeks before your orientation date. If you are 18 years of age or older, you may use SOLAR to submit the response form.

Complete my Student Health Form

Send in a completed Health Form (Health History and Physical) before the first day of classes.

Complete the Required Health Insurance To Do Hold

All full-time matriculated students must have health insurance coverage at all times without exception. Stony Brook offers a health insurance plan for all full-time domestic students, but you may waive this insurance plan for one of your own choosing.

To acknowledge this requirement and either fill out a a waiver form or accept the university's student health insurance plan, please log into SOLAR and complete the following instructions:

1. Log-in to SOLAR
2. Click on "details" under the "Holds" section in the upper left corner of your home screen.
3. Click on the Hold Item labeled "Mandatory Health Insurance"
4. Answer the subsequent questions as they pertain to your plan for health insurance
5. Click Submit and this hold will be removed

You must complete the above steps before you arrive at orientation.
