Stony Brook would like to recognize all student organizations, sports clubs, and individuals within a club or organization who have received recognition from local, regional, national, or international organizations beyond Stony Brook. The award received must relate to the work within your club, organization, or team.  Selected submissions will be highlighted at the Student Life Awards on Wednesday, May 4 at 5:30pm in SAC Ballroom A.

Please use this form to submit information regarding awards received between April 2010 and April 2011.  The deadline for submission is Friday, April 22, 2011. All fields must be completed.
