Forms
REQUIRED FORMS
If you are an admitted freshman/transfer student, the following forms are required:
- The Immunization Form must be submitted at least two weeks prior to your Orientation date.
- The Meningitis Form must be submitted at least two weeks prior to your Orientation date. If you are 18 years of age or older, this requirement can be met by logging on to your SOLAR account and submitting your response electronically under "Holds."
- The Health Form must be submitted before the first day of classes.
If you are a student athlete:
- The Immunization Form and Health Form must be submitted at least two weeks prior to your Orientation date.
- The Meningitis Form must be submitted at least two weeks prior to your Orientation date. If you are 18 years of age or older, this requirement can be met by logging on to your SOLAR account and submitting your response electronically under "Holds."
If you are an international student:
- The Immunization Form must be submitted shortly after you receive notification of your acceptance to Stony Brook University. This form should be received no later than 2 weeks before you arrive on campus for orientation.
- The Meningitis Form must be submitted shortly after you receive notification of your acceptance to Stony Brook University. If you are 18 years of age or older, this requirement can be met by logging on to your SOLAR account and submitting your response electronically under "Holds."
- The Health Form must be submitted shortly after you receive notification of your acceptance to Stony Brook University. This form should be received no later than 2 weeks before you arrive to campus for orientation.

If you are a Graduate or SPD student:
