How To Use the Insurance Plan and Claim Benefits

Always carry your HTH Worldwide insurance ID card with you, and make sure you show the card to medical personnel in the hospital or doctor’s office. If you have not received your HTH ID card, then call the Health Insurance Office on campus (631-632-6376) to have a temporary ID card sent to you.

You should read your insurance booklet, which explains how the insurance plan works. This plan permits you to visit any doctors, hospitals, and other medical providers in any country; and covers you for some medical bills in your home country. Always first visit a physician in the west campus Infirmary (SHS) for each new medical condition, if it is possible, as this will waive your $50 deductible. After a first visit to the SHS , you may go to any doctors, hospitals, laboratories, etc. outside the Infirmary, for that medical problem. Call (631) 632-6740 to make an appointment in the Infirmary.

If you believe your medical issue is severe (such as a broken bone; bleeding that will not stop; chest pain), then go immediately to the closest hospital emergency room. Visit the Infirmary as soon as you can afterwards, to report the emergency treatment you received, and to pick up claim forms. If you obtain treatment for a non-emergency sickness when away from the campus, and you cannot visit the SHS first, you must pay the $50 deductible to your medical provider(s).

If you are enrolled as an OPT participant in the insurance plan, your deductible for each medical problem is $50. This cannot be waived, and you cannot use the Infirmary at all.

Please note that this plan uses a ‘preferred provider organization. Go to www.aetna.com and look for “Open Choice PPO” to see the list of medical providers in your geographical area. By using the PPO (preferred provider network) you will definitely save money on your bills.

Go on line to www.hthstudents.com, to set up a personal account, to print out a duplicate ID card if you need one, and to print out a claim form. There are two claims forms: one is for prescription medicines; the other one is for all medical treatments.

If you have received a bill from a medical doctor or hospital, or have paid for a prescription, then complete the correct claim form and attach a copy to your bill. The bill MUST indicate:

  • Your name
  • The medical provider’s name, license number, and address
  • The date of the treatment
  • The coding for your medical problem, and coding for the treatment
  • How much you were billed, and what you paid

Send the bill and claim form to HTH as soon as you can; it takes four to five weeks for bills to be paid. If you paid the bill, then the reimbursement will be sent to you. If you have not yet paid your medical provider, then HTH will pay the medical provider and send you a notice letting you know how much was paid on your behalf.
