Forms and Documents
You will need to bring the following forms and documents to the Office of Veterans Affairs in order to start receiving educational benefits.
Post 9/11 GI Bill (Chapter 33):
- Certificate of Release or Discharge from Active Duty (DD Form 214)
- Application for VA Education Benefits (VA Form 22-1990)
- Stony Brook University VA Benefits Enrollment Application (SBU VA Form)
- A copy of your class schedule.
Montgomery GI Bill - Active Duty (Chapter 30):
- Certificate of Release or Discharge from Active Duty (DD Form 214)
- Application for VA Education Benefits (VA Form 22-1990)
- Stony Brook University VA Benefits Enrollment Application (SBU VA Form)
- A copy of your class schedule.
Montgomery GI Bill - Selected Reserve (Chapter 1606):
- Certificate of Release or Discharge from Active Duty (DD Form 214)
- Notice of Basic Eligibility (NOBE) (DD Form 2384 or DD Form 2384-1)
- Application for VA Education Benefits (VA Form 22-1990)
- Stony Brook University VA Benefits Enrollment Application (SBU VA Form)
- A copy of your class schedule.
Reserve Educational Assistance Program (Chapter 1607):
- Certificate of Release or Discharge from Active Duty (DD Form 214)
- Notice of Basic Eligibility (NOBE) (DD Form 2384 or DD Form 2384-1)
- Application for VA Education Benefits (VA Form 22-1990)
- Stony Brook University VA Benefits Enrollment Application (SBU VA Form)
- A copy of your class schedule.
Survivors' and Dependents' Educational Assistance Program (Chapter 35):
- Dependents' Application for VA Education Benefits (VA Form 22-5490). This form must be filled out by the dependent and the veteran.
- Stony Brook University VA Benefits Enrollment Application (SBU VA Form)
- A copy of your class schedule.
